In 1989, Piana won the NPC Mr. Teen California competition. When he was 18, Piana began a typical steroid cycle known as “test and deca,” which mixes testosterone and Deca-Durabolin. He said in 2014 that he became “hooked” on steroids after seeing remarkable results.  He was named NPC Mr. California in 1998.  For the next 25 years, he competed on and off, winning NPC tournaments in the 2003 Los Angeles Super-Heavyweight category, the 2009 Sacramento Super-Heavyweight division, and the 2009 Border States Classic Super-Heavyweight division and overall title.  He was on the covers of Ironman magazine in November 1998 and Muscle Sport magazine in Summer 2015. 

Rich Piana quite competing professionally and started a nutrition product line called 5$ nutrition. 

Piana died on August 7, 2017 while having a hair cut. The reason of his death is sudden major heart attack.
